About Your Visit and Forms

Please arrive 15-30 minutes early for your appointment to allow you time to complete, or update, the necessary forms.

When you call to schedule your appointment, you may request to have these forms mailed to you in advance.

Prescription Refills

Please call your pharmacy so that the pharmacy may fax the prescription request to us.

Insurance

The receipt given to you after each visit has been designed to be used for insurance purposes. All claims for surgical and obstetrical care will be filed by our office staff. Please remember that your insurance is a contract between you and your insurance company. You are responsible for your bill regardless of what your insurance company pays, however, this office will make every effort to assist you in receiving the maximum benefit to which you are entitled.

Heres what you need to know about your health insurance coverage before seeking treatment:

  • What is covered vs. what is not covered
  • What are the deductibles and/or co-payments
  • What are your in network labs and imaging centers
  • What are your pharmacy benefits and in network pharmacies
  • What approvals, if any, are necessary before you go to a doctor or hospital or if you require surgery and diagnostic tests
  • Who provides the approvals and how are referrals to a specialist handled
  • What hospitals are included in the network and are hospital stays limited by the insurance Company

Dr. James Cawley participates as a specialist in most insurance and Health Care Plans available in this area. We encourage patients to contact their insurance customer service to become familiar with their benefits and rules pertaining to their care at this practice before making an appointment. We do work closely with our patients to optimize their insurance benefits. As we accept many insurance plans, please contact our office at 940-327-8505 to find out if yours is one of them.

Our office files insurance claims only when we are able to verify insurance benefits. Otherwise, we collect payment in full at the time of service and give you an itemized statement in order for you to follow-up directly with your insurer. We file Medicare patient claims as required by the federal government and will accept Champus / TriCare and Medicaid patients.

Fees and Payments:

This office will make every effort to keep the costs of your medical care down. Patients are expected to pay their bills in full at the time of service unless previous arrangements have been made. Patient bills may include co-payments, co-insurance, deductibles, and charges for non-covered / non-insured services. Patients may pay by cash, check, MasterCard, Visa and American Express. Upon verification of insurance benefits, we file insurance claims. The staff also works closely with self-pay patients and those with large balances to set up payment schedules that meet both the patient and practice needs. Our fees are within the customary range for this area and reflect the level of care you receive and the high level of training by Dr. Cawley.

The fees for obstetrical care include all prenatal visits, delivery, one ultrasound, and the six weeks check-up following delivery. Should the need arise for extra testing such as additional ultrasounds or fetal heart monitoring, the appropriate charge would be added.
